I, JOHNSON YESAYA MGELWA, Adult male, Christian of Sinza Ward, Ubungo District, Dar-es-salaam Region in Tanzania, do hereby affirm and state as follows:

1. That I’m JOHNSON YESAYA MGELWA, the holder of Account No. 08898500 registered at CRDB BANK PLC, MLIMANI CITY BRANCH, DAR-ES-SALAAM.

2.That the said Account has been dormant for a long time and I want to reactivate the same.
