AFFIDAVIT OF DATE OF BIRTH 

I AMINA DAUDI LOLO, Female adult, Muslim of Kimara ward, Ubungo District, Dar es Salaam Region in Tanzania, do hereby affirm and state as follows: 

1. That I am a biological Mother of KASSAABU MALIKI TWALIBU.

2. That the said KASSAABU MALIKI TWALIBU Was born at Regency Medical Center, Ilala District, Dar es Salaam Region, on 30th day of December, 2018 as it appears in the Hospital form No. 3843966.

3. That the date of birth of my son is wrongly mentioned in the Birth Certificate No. 1002566714 which is 19th December, 2018. 

4. That any document bearing this date 30th December, 2018 should be acted as a deed of correct date of birth of my son.  

5. That what I have started in paragraph 1, 2, 3, 4 and 5 is above is true to the best of my own knowledge and belief.
